Medical bills are the #1 cause of bankruptcy in America, around 1 in 3 and growing.
Yet fewer than 1% of patients even try to appeal, despite an ~80% success rate.
At Dorsal, weāre building infrastructure to fight backā think Rocket Money meets revenue cycleāreturning $60 billion in wrongful charges to the people and clinics who need it most.
Dorsal (adj.): of or relating to the back
Weāre the only healthcare company thatās truly got your backāthe backbone for the future of appeals and community-driven transparency.
Hi, Iām Abrar, founder of Dorsal.fyi. Weāve been working in stealth for just over a year, and weāre finally ready to share what weāve been building.
Last week, the House passed a $1 trillion cut to Medicaid. Rural hospitalsāmany still underwater post-COVIDāare about to face an existential crisis. ~25% of folks nationwide are enrolled on Medicaid, and for certain communities the rate can be over 66% of their patient base. The math doesnāt pencil out anymore.
My dad is a bootstrapped founder. My mom is a medical assistant. I grew up splitting the differenceālearning how systems scale from one parent, and what it means to truly serve from the other. This foundation led me through eight years in healthcare AI, from building EHR systems for dialysis centers in Bangladesh to working on the generative AI team at Epic, where I watched technology that could heal being used to obscure and extract.
Lazy phone agents that try to flip consumers on an individual basis will never change the incentive structures inherent to the problem. Weāre building infrastructure that cuts across the administrative fat and puts patients first.
š„ The $60 Billion Opportunity
Hereās the reality: only 0.5% of Americans appeal their medical bills. But when they do, they win 70ā80% of the time. Thatās tens of billions left unclaimed every year by patients and providers who simply donāt have the tools nor the time to fight back.
At Dorsal.fyi, we use AI to identify and overturn improper chargesāfrom inflated billing codes to unbundled proceduresāand return that value to patients, clinics, and care plans. Every bill we process makes the next one smarter. Every dollar we return builds a flywheel of trust, data, and margin.
šÆ More Than a Grassroots Movement
Dorsal started with a simple question: why do patients need a PhD to understand their bill? And how can the hundreds of millions of honest folks who keep America running possibly stand a chance?
Weāre building something bigger than a grassroots movement. Dorsal.fyi is a new model for how patients engage with the revenue cycle. Our platform educates and activates patients to self-advocate, which directly improves care access, reduces no-shows, and supports healthier margins for providers under todayās care models.
By embedding directly with clinics, care plans, and hospitals, especially those serving vulnerable populations. Weāre the financial OS for patient engagement. You, your neighbors, investors, and seasoned administrators alike may simply succumb to this culture of apathyā but we will never give up. The job is not finished and it may never fully be, but weāre gonna try our damnedest to get there. We cannot rest until our data-driven solutions actually work and reach each and every American.
ā” Why Now, Why Us
Patients are bearing more costs than ever, but theyāve never had less power. Transparency laws havenāt solved the problem. Traditional RCM players have no incentive to help the people footing the bill.
Our team includes the former CMO of Texas Medicaid at Cigna, a former sales VP at Oracle Health and McKesson, and engineers with decades of AI experience inside academia and healthcareās largest systems. We are NOT some McKinsey bros cosplaying as activists. Weāre builders committed to the messy work of attacking issues that legacy players and GPT wrappers alike wonāt touch.
This is more than just good business. We are no moonshot. Itās an inevitable infrastructure for the future of care.
In five years, āDorsalingā will be as obvious, widespread, and straightforward as Googling.
